Illegal Immigrant Attestation

Vendors wishing to provide supplies or services to the University of Tennessee must first complete and return the University of Tennessee's Attestation Form  to the soliciting Purchasing Department.  The form may be returned either through the regular mail or by facsimile.  Any purchase order received by the vendor is contingent upon receipt of this completed form by the Purchasing Department.
